Getting home, and getting back to yourself
Coming home from hospital is a relief and a risk at the same time. The first days and weeks are when confidence is most fragile, when falls happen, and when families are most stretched. Reablement support is designed for exactly this window.
It is different from ongoing care. The goal is not to do things for someone indefinitely, it is to work alongside them for a few weeks so they relearn the routines, rebuild strength and confidence, and need less support at the end than at the start.
We can be part of a discharge plan arranged with the hospital or social services, or arranged privately by a family who want the first weeks at home to go well. Either way, support starts with a clear goal and an end point in view.
What reablement support includes
Being there on day one
Support in place for the journey home and the first hours back, when everything feels hardest.
A safe home to return to
Basics ready before discharge: food in the fridge, the bed made, the heating on, and obvious trip hazards dealt with.
Rebuilding the routine
Working alongside someone as they wash, dress and cook again, doing things with them rather than for them.
Regaining confidence
Encouragement to move, practise and try again after a fall or illness, at a pace that is safe.
Medication support
Prompting and support with new or changed medication where this has been assessed and authorised within the care plan.
Watching for problems
Noticing signs of infection, poor eating or declining mobility early and flagging them to family and, where appropriate, the GP.
Working with professionals
Following guidance from physiotherapists and occupational therapists so home practice matches the professional plan.
Stepping back on purpose
Reviewing support weekly and reducing it as independence returns, because that is the point.
How reablement is different
A goal, not a rota
Support starts with what the person wants back: the stairs, the kitchen, the garden, the walk to the shop. Everything works towards it.
Do with, not for
It is slower to stand beside someone while they make their own tea. It is also how ability comes back.
Short term by design
Most reablement runs for a few weeks. If longer term support is needed, we say so early and plan it properly.

With consent, we tell families honestly how recovery is going, including when it is slower than hoped.
